Most Falls Are Preventable. So What Keeps Happening?

Falls continue to be the leading cause of fatalities in construction, making fall protection training and prevention one of the most important safety priorities on any job site.

Each year, the Occupational Safety and Health Administration (OSHA) encourages employers to pause during the National Safety Stand-Down to Prevent Falls and refocus on safe practices.

It’s a valuable effort. But preventing falls takes more than a single conversation or annual event.

According to OSHA, falls account for roughly one-third of all construction fatalities.

Most of those incidents are a result of a single misstep, but multiple missed details that go unaddressed and lead to an incident.

And when those gaps lead to an incident, the impact goes far beyond the immediate injury. Lost productivity, project delays, and long-term costs can escalate quickly.

Why this keeps happening

Fall incidents tend to follow the same patterns.

  • Training that was completed but not reinforced.
  • Equipment that’s available but not fully understood.
  • Hazards that become easier to overlook over time.

This is where complacency starts to take hold.

When a task feels routine, risk can fade into the background. Team members rely on familiarity instead of actively evaluating the situation in front of them.

That shift is subtle, but it’s where many incidents begin.

Where fall protection programs lose effectiveness

Most organizations have the right elements in place. Training has been delivered. Equipment is available. Inspections are documented.

But job site day-to-day operations don’t always reflect that.

Common tie-off mistakes

  • Workers tied off to points that are not designed to support a fall, such as fire suppression pipes, conduit, scaffold rails, or mobile equipment
  • Overhead crane structures used as anchor points without proper lockout/tagout or verification
  • Fall protection equipment available but not consistently used
  • Harnesses worn incorrectly or not properly fitted
  • Limited understanding of the difference between certified and non-certified anchor points

Anchor point confusion

Not everything you can attach to will hold during a fall.

When that distinction isn’t clear, equipment creates a false sense of protection instead of actually reducing risk.

Fall protection training that supports real decisions 

Training is required under OSHA standards, including OSHA 1926.503 Fall Protection Training Requirements. But meeting the requirement isn’t what prevents incidents.

What matters is whether workers can apply that training in real time.

Training is more likely to stick when it includes both classroom instruction and hands-on demonstration. Seeing what actually happens during a fall changes how people think about their equipment and their decisions.

What workers should be able to do:

  • Recognize when fall protection is required
  • Understand the limitations of their equipment
  • Identify appropriate anchor points
  • Properly don, fit, and use their harness
  • Make safe decisions under changing job site conditions

Without reinforcement, training becomes something that was completed rather than something that’s actively used.

Inspections only work if they’re done with intent

Fall protection equipment must be inspected regularly. OSHA requires that equipment be maintained in safe condition and removed from service if damaged.

In practice, inspection programs don’t always function as intended.

Some common issues include:

  • Pre-use inspections not consistently performed
  • Webbing and components not fully checked
  • Damaged or worn equipment remaining in service
  • Equipment shared without clear accountability

It’s not always that inspections are ignored. They’re often rushed or treated as routine. Inspections should also be documented to ensure a clear record of equipment condition and accountability.

When that happens, problems go unnoticed until the equipment is needed most.

A rescue plan isn’t optional

Having fall protection in place is only part of the equation.

Calling 911 is not a rescue plan.

If a worker falls and is suspended, time becomes critical. Without a plan in place, response is delayed and risk increases.

What every rescue plan should address

  • How a worker will be reached
  • Who is responsible for initiating rescue
  • What equipment is available on site
  • How quickly the rescue can be performed

Rescue planning is often overlooked, but it’s a key part of a complete fall protection program.

What to review during Safety Stand-Down week

The Stand-Down is a good opportunity to step back and evaluate your program.

Focus on what’s actually happening on your job site:

Training

  • Are workers trained under current standards?
  • Can they apply that training to the work they’re doing today?

Equipment

  • Is fall protection gear in good condition?
  • Is it being used consistently and correctly?

Inspections

  • Are pre-use inspections happening?
  • Are they thorough, not rushed?

Hazard recognition

  • Are fall hazards identified before work begins?
  • Are controls in place and followed?

Real-world application

  • Can workers demonstrate proper use of equipment?
  • Are safe practices consistently followed?

Make it more than a moment

The Stand-Down brings attention to fall prevention. What matters is what happens after.

Reducing risk requires:

  • Ongoing training
  • Consistent and thorough inspections
  • Active enforcement from supervisors

Accountability across the crew matters. When something doesn’t look right, it needs to be addressed in the moment. A reminder, a second look at a tie-off point, or speaking up when something doesn’t look right can prevent an incident.

At the end of the day, the goal is simple. Everyone goes home in the same condition they came to work in.

Preventing falls protects your people and avoids the kind of operational and financial impact many companies underestimate.
